Support Coordination

We offer quality support coordination of all levels.

A Support Coordinator is a professional who assists NDIS (National Disability Insurance Scheme) participants in identifying and coordinating the support and services they need to achieve their goals and enhance their quality of life. The services a support coordinator offers to NDIS participants are designed to help them navigate the NDIS system, understand their funding, and connect with the right services and supports.

Here are some of the services a Support Coordinator may offer to NDIS participants:

  1. Developing a support plan: Support Coordinators work with NDIS participants to identify their goals, preferences, and needs. Based on this, they develop a support plan that outlines the necessary supports, services, and resources required to achieve these goals.

  2. Connecting with service providers: Support Coordinators can assist NDIS participants in finding service providers that meet their needs. This may involve researching and identifying suitable providers, obtaining quotes, and arranging appointments.

  3. Coordinating support services: Support Coordinators help NDIS participants coordinate the services they require. They may work with service providers, therapists, and other professionals to ensure that the participant's needs are met and that their services are delivered in a coordinated and effective manner.

  4. Managing NDIS funding: Support Coordinators help NDIS participants understand their funding and how to use it to achieve their goals. They may assist in managing the participant's budget, ensuring they remain within their funding limits and maximizing their use of available funds.

  5. Advocating for participants: Support Coordinators advocate for NDIS participants to ensure that their rights are respected, and they receive the support they need. They may liaise with government agencies, service providers, and other stakeholders on behalf of the participant to resolve issues or concerns.

Overall, a Support Coordinator is an essential resource for NDIS participants, helping them to navigate the complex NDIS system and connect with the support and services they need to achieve their goals and improve their quality of life.

Coordination of Supports (Level 2) is an NDIS support item that aims to empower participants in building and designing their supports. It emphasizes linking the broader systems of support across a complex service delivery environment. The goal is to support participants in directing their lives, not just their services, by building and maintaining a resilient network of formal and informal supports. Coordination of Supports involves working together with the participant to understand their funding, identify their expectations from services, and how they want these designed. The support also includes coaching the participant and developing their capacity and resilience in their network. The service encompasses several elements, such as understanding the plan, connecting with supports and services, designing support approaches, establishing supports, coaching, refining and reflecting, targeted support coordination, crisis planning, prevention, mitigation, and action, building capacity and resilience, and reporting to the NDIA.
